A person. First name, last name and NPI on every row. The credential is on nearly all of them, and the practice they bill from and its phone number on most. If you sell to allergists, you can write to the allergist who signs the order instead of whoever reads the shared inbox.
No verified count yet. This file's email column is built after you order and reaches you within a day at no extra cost, and only addresses that pass an SMTP check go in. Expect a minority of rows to get one: in our first test run, 55 of 400 cardiologists got a verified email (Sep 10, 2026). Every row keeps the practice city, state and ZIP. Yes. CMS enrolls allergy and immunology under a single specialty, so clinical immunologists appear here alongside allergists. Pediatric allergists who enroll under this code are included and their practice names usually say so. What you will not find is a separate immunology only cut, because the source data does not support one.
