The CMS Doctors and Clinicians national downloadable file, the Medicare enrollment roster, version Aug 18, 2026. Every urologist in it treats Medicare patients, which is a filter on its own: no lapsed enrollments, no phantom practices. Emails are added and verified by us, not bought from a broker.
Not on the verified rows. The email column is the urologist's own work address, matched to the practice domain and checked against the mailbox. The practice phone sits in its own column for the times you do want the front desk.

Yes. Every row has city and state, so you can cut the file to a region before your first send. California holds the largest share of the urologists in this list. There is no geography surcharge and no minimum order: you pay $295 for the national file and slice it however you want.
Role addresses, catch-all-only domains and dead mailboxes are removed before you get the file, which is what keeps a cold send from wrecking a domain. If hard bounces still pass five percent on the verified column, we refund the $295 against your bounce report.
