One row per NPI in the CMS Doctors and Clinicians file dated Aug 18, 2026, for specialties where at least 90 percent of clinicians hold an MD or DO, placed in the state of their main practice location. Physicians who bill no Medicare patients are not in the file. Nurse practitioners, physician assistants, chiropractors and optometrists are not in these totals.
